HEAT target:Psychiatric Readmission Data

To reduce the number of readmissions (within one year) for those that have had a hospital admission of 7 days by 10% by the end of December 2009.

What do we mean by readmission?

The basis of the readmission is based on individual patients who have an admission of at least 7 days to a psychiatric hospital or designated psychiatric bed and then are readmitted to a psychiatric hospital or designated psychiatric bed in Scotland for at least a further continuous 7 days within 365 days of discharge from the original admission.

What is the baseline?

The baseline for this target is being taken as December 2004 The measure specifically relates to the number of people who have been readmitted within 1 year of discharge. Each readmission is allocated to the Health Board of residence as at the date of 1st discharge".

What is the reporting period?

All spells of treatment that are identified as starting with a readmission end with a discharge that may precede a further readmission within 365 days. Discharge/ re-admission pairs are allocated to a 12 month reporting period on the basis of the discharge date.

If a patient has more than one readmission within the 12 month reporting period only the first is counted.
