1. What do we mean by Care Management?
Care Management is:
·A process in which an individual's needs are assessed and evaluated, eligibility for service determined, a care plan prepared and implemented, services tailored to individuals needs provided, and needs are monitored and re-assessed;
·focussed on people with complex, or frequently or rapidly changing needs; and
·undertaken by a range of professionally qualified staff in social work and health, with appropriate training, skills and experience.
Original guidance on care management was developed in 1991, and a further update was released, in 2004, with the aim being to refocuses care management on people with complex or changing needs and on extending the range of professionals undertaking care management.
During 2009, a small reference group was convened to ensure the Scottish Government's focus on outcomes was reflected throughout the Care Management Training Framework. Led by the Partnership Improvement and Outcomes Division, and including input from both local authority and Community Health Partnership representatives, this latest version develops the messages contained in both the original guidance and 2004 update.
