Joint Improvement Team Publications

The Joint Improvement Team have developed a number of toolkits to assist partnerships. These include:

• A raange of toolkits and reports on Delayed Discharge which are available at http://www.jitscotland.org.uk/action-areas/delayed-discharge/
• Commissioning Effective Services for Older People Workbook. This online workbook has been designed to help local authority and health partners gather data across the whole system to assist capacity planning. It will be of particular use when considering older people's services, as local commissioning plans are developed.

• Flowchart to support the use of evidence in commissioning services for individuals with learning disabilities. This online toolkit / flowchart has been designed to help commissioners use existing evidence and good practice when developing and evaluating individual service plans for people with learning disabilities.
