VISIBLE, ACCESSIBLE AND INTEGRATED CARE

COMMUNITY HEALTH NURSE (DRAFT) JOB DESCRIPTION

As you will be aware work has recently been undertaken to develop a draft job description to support the development of the new discipline of Community Health Nurse (CHN), and I am pleased therefore to provide a draft job description for consideration.

The draft job description has been developed using the outputs from a number of staff engagement events which have taken place across Scotland. These have enabled staff to be involved in developing the vision and components of the CHN role, and it has been underpinned using the capability framework. The process for developing the job description is illustrated on the attached flow chart.

The next stages will include development of a KSF outline and AFC profiling in one of the development sites.

We are very keen to receive feedback and I would be very grateful if you could circulate these documents to appropriate stakeholders within your department/ organisation for review and comment.
