"Applying Quality Improvement Science to Performance Challenges"
Wave 5 News (2008)

The Improvement and Support Team launched Wave 5 of the Scottish Primary Care Collaborative (SPCC) in March 2007, with 62 GP Practices. Wave 5 continues to improve patient access to a Primary Health Care professional and further develop the chronic disease model. In Year 1 practices worked to improve outcomes for people with a diagnosis of Chronic Obstructive Pulmonary Disease, including Asthma (COPD). This work will continue in Year 2, however in addition, practices will also focus on improving outcomes for people with Chronic Kidney Disease (CKD). Practices will begin to record their CKD baseline figures in February 2008.

The 3rd Learning Workshop was held on 18th & 19th March 2008, where practices shared their improvements from Year 1 and focused on the improvement work ahead of them in Year 2.

Phase I Report (January 2008)

The SPCC Phase I report highlights the achievements made by GP practices in improving patient access to a Primary Health Care professional and improving outcomes for people with Diabetes.
