Partnership Improvement and Outcomes Division

Overview

A constituent part of the Directorate of Primary and Community Care within the Scottish Government's Health Directorates, the Partnership Improvement and Outcomes Division (PIOD) was established around two complementary core themes, "Outcomes" and "Improvement", both set within a context of partnership working which reflects the Scottish Government's strategic requirement to work in partnership across organisational and professional boundaries. PIOD brings together many key strategic elements of the partnership between NHSScotland, the Scottish Government and COSLA.

Purpose, Aims and Approach

PIOD's key purpose is to enable health, housing and social care partnerships to deliver better outcomes for service users and carers. The aim is to build capacity within local partnerships and create a performance management infrastructure that both supports and challenges partnerships in their pursuit of continuous improvement. PIOD's approach is to actively engage with those organisations and groups with a stake in Health, Housing and Social Care in order to design, develop and deliver improvements to the quality, range and responsiveness of services.

Improvement

Improvement work within the division is led by the Joint Improvement Team working closely with a policy team focussing on Delayed Discharge and NHS continuing healthcare. In addition the team is involved in revising guidance on Equipment and Adaptations. Equipment and Adaptations play a vital role in improving the quality of people's lives. They can help people to live more independently in their own homes and can reduce the need for home care services.

Outcomes

The Joint Outcomes Team (formerly Joint Future) focuses on policy development leading to more effective, joined up working between local authorities and the NHS in community care, with better outcomes for patients, service users and carers. Specifically, the team's remit currently includes Shared Assessment, Care Management, measurement of outcomes for users and carers via the Community Care Outcomes Framework and Talking Points, and the Integrated Resource Framework, which is a key programme of work within Shifting the Balance of Care.
