APPENDIX II: SELF ASSESSMENT TEMPLATE - STANDARDS AND CRITERIA

This section provides a tool to enable NHS organisations to audit current practice against the standards criteria detailed in the report.

Part 1: Patient Safety and Clinical Governance Standard

The NHS in Scotland must have systems in place to support the safe and secure prescription, supply and administration of medicines in accordance with legislative requirements, national guidance and best practice.

2.2.4 The prescribing, supply and administration of medicines is undertaken by appropriate healthcare staff.

2.2.5 The system for the prescribing, supply and administration of medicines permits audit of personnel, procedures, policies, environments and facilities.

2.2.6 There is a medication incident recording system in place.

Part 2: The Patient Experience Standard

The NHS in Scotland will make medicines available to meet the clinical needs of patients and wherever possible actively involve patients, parents and carers in making decisions about their medicines and that of their children.

2.2.7 To support safe prescribing a comprehensive medication review is an integral component of a hospital admission.

2.2.8 Patients are encouraged to actively participate in prescribing choices and decisions about their medicines.

2.2.10 To support patient empowerment and aid continuity of care, patients should have their own individually dispensed supply of medicine.

2.2.11 Patients are supported and encouraged to take an active role in managing their own care and to take responsibility for their medication including, where appropriate, self-administration of medicines.

2.2.12 Medication to be continued after discharge from hospital is planned with the patient/carer.
