About this questionnaire

All of the questions in this questionnaire are based on what people have said is important to them about assessments, that is the discussions people have with a member of health or community care staff about the kind of help and services they need. These discussions may have been held at different times and in different places (for example, in hospital and at your home).

The questions are divided into 4 sections, which match what people have said is important to them. These sections are

  1. Information and preparation for the assessment meetings

  2. How your assessment was conducted

  3. More about what was discussed

  4. What the assessment meeting led to.

Each section starts with a few general questions. These questions sum up what people have told us is most important to them about assessment discussions.

Each section also has several more detailed questions. These also reflect what is important to many people, but not all of them may be equally important to everyone.

All of these questions are about points which health and social services want to know if they are getting right, and if not how they can improve what they do.

We understand that people's experience and reaction to their assessments will be very varied. The questions are to help us find what mattered to you. They are also to help us find out what is working well as well as what isn't working so well and could be improved.

The questions refer to the person or people who did your assessment discussions as "the assessor". This means the person (or people) who came to have the meeting(s) with you.

Please answer as many questions in each section as you feel are relevant to you.

The questions have tick boxes asking you to say 'yes' or 'no'. Please tick the appropriate box for each question. If you feel your answer should be 'yes' and 'no' please explain this in the 'Your Comments' box. We would also welcome any other comments you would like to make; please write these in the 'Your Comments' boxes.

Your answers will be treated as strictly confidential. This means your answers will only be seen by the Infusion Co-operative research team. No one else will see them. We won't use your name or anything else that might identify you in reports or other public documents about this research.
